Sleep is not a passive state; it is a highly active neurological and metabolic process. During the night, the glymphatic system flushes neurotoxic waste products—including amyloid-beta plaques associated with Alzheimer's—from the brain.
The Two-Process Model
Sleep regulation is driven by two mechanisms: Process C (the circadian rhythm) and Process S (sleep pressure).
Process S builds up throughout the day via the accumulation of adenosine, a metabolic byproduct. Caffeine works by blocking adenosine receptors, artificially masking sleep pressure. Process C is regulated by the suprachiasmatic nucleus in the brain, which relies heavily on light exposure to dictate melatonin synthesis.
Clinical Protocols for Insomnia
- Light Management: View natural sunlight outside within 30 minutes of waking. This sets the circadian clock. Conversely, block blue light (screens) at least 90 minutes before bed to allow the pineal gland to release melatonin.
- Thermoregulation: Core body temperature must drop by approximately 2°F to initiate sleep. Keep the bedroom cold (65°F / 18°C) and consider a warm shower before bed—the subsequent rapid cooling of the body accelerates sleep onset.
- Consistency: Waking up at the exact same time every day, regardless of how poorly you slept, is the single most effective way to anchor your circadian rhythm.
